30 days with no service or setup charge
We’re choosing five families to try companionship and help at home for 30 days. Those families pay no service or setup charge during that time.
Send your request by November 16, 2026. Visits begin December 1, 2026. We may close requests earlier once the five families are chosen.
If you want visits to continue after 30 days, we’ll agree on the help, schedule, and price. Groceries and other household purchases are separate and must be agreed in advance.
First, we talk with you and your parent about the help they want. Sending this form doesn’t book visits or commit you to paying.
